Guest Additions break visual output in Gens emulator

Description
After installing guest additions into a VM, the output of Gens (a Sega Genesis emulator) and it's derivatives (DGens, GensKMod, Gens/GS) no longer shows properly. The screen shows 2 squashed images side-by-side, with incorrect colours.
Without guest additions installed, the output is fine. This effect occured when using Windows XP & 7 as the Guest, on a Debian host.
