To be honest, I don't quite know when this will happen. The normal case is to just show that the hard disk is not accessible, even if there are saved states involved.

I've had it happen because I was sharing the same .VDI file for two different virtual machines. However, I was always very careful not to run both guests at the same time.
There's some kind of conflict here that confuses VirtualBox.
In the forum thread linked above I have uploaded my configuration files.
Could you please take a look at them to see if there's anything suspicious inside?

Closing, please reopen if still relevant with a recent release of VBox.
VirtualBox behavior is excessively intolerant. It should be more understanding to its users and give them the opportunity to specify the path to the .VDI file.
