Description
I have a VM in safed state made with 4.2.10 and after upgrade to 4.2.12 I got

Thanks for the report, we have found and fixed this issue now.
Here is a test build containing the fix. Please give it a try and let me know whether the issue is solved with it:

I'm having the same issue. Just upgraded to VirtualBox 4.2.12 r84980 on MacOS 10.8.3 and my Ubuntu 12.10 VM fails to start with:
Failed to open a session for the virtual machine Ubuntu 12.10. Failed to load unit 'HGCM' (VERR_SSM_LOADED_TOO_MUCH).
Result Code: NS_ERROR_FAILURE (0x80004005) Component: Console Interface: IConsole {db7ab4ca-2a3f-4183-9243-c1208da92392}

Dear moderator, iam Atif i have just installed V.B version 4.2.12 downloaded from www.virtualbox.org on my Windows 8 Pro Laptop. The issue i am facing is that iam not able to create a Virtual Hard Disk ie .vhd on Win 8. i installed virtual box on different desktop with Windows 8 Enterprise, but same issue still remains. This Virtual Box 4.2.12.84980 works good on old WIN XP its much peppy and lite. Please resolve the issue. when i create Virtual Hard Disk on win 8 it takes unlimited time to create although the .vhd hard disk shows inside the virtual machine folder the default location but it is inaccesible by the virtual machine.

I am having the same problem with OSX 10.7.5 host trying to boot from a Windows 8 install CD-ROM. The OSX.dmg link above no longer works.
Failed to open a session for the virtual machine Windows8.
Failed to load VMMR0.r0 (VERR_SUPLIB_WORLD_WRITABLE).

I hit this today. Mac OS X 10.9.4, VirtualBox 4.3.8 r92456. It happens with all 3 snapshots.
Failed to open a session for the virtual machine BLAH.
Failed to load unit 'HGCM' (VERR_INVALID_PARAMETER).
Result Code: NS_ERROR_FAILURE (0x80004005) Component: Console Interface: IConsole {8ab7c520-2442-4b66-8d74-4ff1e195d2b6}
The one suggestion I found for recovery was to stop everything VirtualBox related, edit the BLAH.vobox file (after taking a copy) and remove any shared folder definitions (leaving only <SharedFolders/>). This did not help.

Please reopen if still relevant with a recent VirtualBox release.
Same issue: MacOSX host, Ubuntu guest.
From VBox.log:
