Also seeing this on Vista 32bit Host running VBox 3.0.1 r55467. Under File->Preferences->Network clicking 'Add host-only network' results in an error of "Failed to create host-only network interface".
I've tried:
- Removing the existing interface and re-adding it.
- Uninstalling and re-installing VirtualBox
- Disabling all adaptors before adding host-only interface
None of which have made a difference.
System:
- Vista 32bit
- 4Gb RAM
- Intel Core2 Duo T9600

Do you have a (default) host-only interface created on install?
Could you enable setupapi logging, as mentioned in "Driver Installation Log" section here, remove (or backup somewhere) setupapi.log, setupapi.app.log & setupapi.dev.log files and try create the host-only interface.
Once creation is complete (i.e. failed in your case), could you attach the newly-created .log files mentioned above to this report?
