Guest Additions 4.1.4 installed
Ubuntu 11.04 Up-to-date no extra packages other than the guest additions from the guest additions .iso installed. ATI Mobility Radeon HD 5470 (Driver version 11.9)
Windows 7 Home Premium 64-bit Intel VT-x enabled Core i3 proccessor (4 cores cores reported, I think it's 2 hyperthreaded to 4) 1 GB ram allocated to guest 2 CPU Guest SMP 8GB .vdi

The following test build should solve the problem:

There is no need to update guest additions, i.e. only host VBox installations needs to be updated.
Please give it a try and let me know how it goes.

I'm still seeing this on a fresh VirtualBox 4.1.14 install. I have a Windows 7 64-bit host and a clean Ubuntu 12.04 guest install with only DKMS and the 4.1.14 guest additions installed. Disabling 3d acceleration lets me see and use the mini-bar.
This issue is caused by ATI drivers bug.
To make a proper work-around, I need to ensure your behavior is similar to what I'm seeing here:
Does the issue goes away when VM window becomes inactive? E.g. while having VM window active & displayed, press <ctrl>+<alt>+<del> (which should be propagated to host, and not propagated to guest), and select "Start Task Manager" in a host popup window.
This should start task manager, make its window active, and VM window inactive.
Is it correct that the issue goes away until VM window becomes active again?
