Description
Section 4.2.1.1 state
- Back in the Virtual Disk Manager, select that ISO file and press the “Select” but-
ton. This will mount the ISO file and present it to your Windows guest as a CD-ROM.
This is not correct, and confusing. The user must use the VDM to add the ISO image and click "OK". Then they must go back to the main xVM window, with the VM powered off, and click on the CD/DVD-ROM item in the Details window to choose which ISO to mount.
For a naive user, this doc error makes it impossible to install the VBoxGuestAdditions.iso tools.

For your original report the user manual describe exactly what to do to mount an iso. I didn't see any errors there. If you use the Devices->Mount CD/ROM... menu entry of your running VM you also get to the VDM. If you add a iso there you have to use the Select button to automatically attach that new iso. Btw, you didn't have to do this all for the Guest Additions. There is a Devices->Install Guest Additions entry in a running VM. Please also note that the menu entries of a running VM & the main VirtualBox selector are different, maybe this has confused you.
There are additional errors in section 4.4.1. Instructions 2 and 3 describe how to add the VBoxSolarisAdditions.pkg file. Step 2 is missing the fact that the user has to cd into the vboxadditions* directory before adding the package:
Step 3 specifies the wrong response to the pkgadd command. The user should simply press Enter, to specify that All packages should be installed.
